Instructions
To qualify for medical marijuana in the state of Maryland, the patient must present a qualified diagnosis by a provider.
You will need a photo ID, and proof of your medical condition which could include: medical records, pill bottle, x-ray, prescription, doctor’s notes
You must then register with the Maryland Medical Cannabis Commission
Once registered and have a 16-digit number you can get evaluated and certified for cannabis at Unique minds in a timely manner
Medical Conditions
Qualifying medical conditions include cachexia, anorexia, wasting syndrome, severe or chronic pain, severe nausea, seizures, severe or persistent muscle spasms, glaucoma, post-traumatic stress disorder (PTSD), or another chronic medical condition which is severe and for which other treatments have been ineffective
